Leaving hospital

For people with disability, and their families, who are getting ready to leave hospital and need support or a place to live.

Tell the NDIS you are in hospital

If you are an NDIS participant, tell the hospital staff. With your consent, they will let the NDIS know. You can also call the NDIS on 1800 800 110.

The NDIS says it will contact you within 4 days of hearing that your plan may need to change. A hospital liaison officer may work with you and the hospital team.

Not on the NDIS? If hospital staff think you might be eligible, they can help you apply while you are in hospital.

Guide to your hospital stay (NDIS)

SIL and SDA

Supported independent living (SIL) is help from support workers with everyday tasks in your home. You may share support workers with the people you live with.

Specialist disability accommodation (SDA) is the home itself, built for people with very high support needs. It is not the support in it.

SIL and SDA providers must be registered with the NDIS Commission. SDA homes must be enrolled by a registered provider. Ask a provider to show you their registration.

If your new home is not ready, your plan may be able to fund a short stay somewhere else for up to 90 days. This is called medium-term accommodation. Ask your hospital team or the NDIS.

Find an SDA vacancy

The NDIS has an SDA vacancy finder. You can search by building type, location and more. Not every vacancy is listed there.
